DMC has developed this Staff Performance Appraisal Form in accordance with standardized best practice for the assessment of the overall performance of every DMC staff. It is intended to meet the requirements of Clause 7.1 of the ISO 9001:2015, which is aimed at identifying areas of commendation as well as areas that require improvement for all staff.
The following consists of the necessary information and guidelines for the conduct of staff performance appraisal in DMC:
- Each staff is expected to go through an electronic copy (or a hard located at the department/unit) of this document.
- This document should NOT be printed indiscriminately or taken out of DMC’s computer as electronic copy.
- After reading and understanding the document, ALL staff are expected to complete the sections A-C of the form and submit. The self-rating of performance (Section B).
- Exaggerated or dishonest self-appraisal can be penalized by suspension, demotion
- The link to the online version of the performance appraisal form will be provided to ALL STAFF.
- The appraisal is conducted two times in a year should be completed ONLY ONCE at the time of an appraisal.
- Closing date for self-appraisal are July 3 or January 3 of every year.
- Heads of Dept/Units will be provided with the outputs of the self-ratings of staff in their dept/unit for reviews and comments.
- All outputs from Heads of Dept/Units will be reviewed and assessed by the EDA, DCS or ELD.
- The CMD or EDA approves the commencement (June 15 and Dec 15 of every year) and final report of staff performance appraisal (July 15 and January 15 of every year).
